In order to verify your caregiver’s status for his/her season pass, please provide us with the requested information shown below.

• The completed form will be emailed to the provided  Employer’s Email for you to print. Once printed, please sign beside Employer’s Full Name and have your caregiver bring the completed and signed form with them when they come in to purchase a membership.
